Suicidal ideation is handled directly and is a significant plot thread: Roman struggles with thoughts that death would be easier and that those around him would be better off without him; this is not glossed over. There is one notable scene where Roman, mid-PTSD nightmare, briefly chokes Lucas before Lucas's voice brings him back — it is unintentional and Roman is horrified; it is flagged in the author's own content warnings.
